Application Process for New Students

The 2026-27 New Student Application is now available!

Your student's application is considered complete when the school receives all of the following items. 

1. Completed 2026-27 Online New Student Application. This application is from the Diocese of Lafayette. An account will need to be created to complete the application if you have not done so in the past.  
2. Non-Refundable Application FAMILY fee of $300
3. Submitted Documents:

• Copy of Student's Birth Certificate
• Copy of Student's Baptismal Certificate, if applicable
• Teacher Questionnaire (for students currently enrolled in another school). Parents are asked to copy/email appropriate link to the current teacher. We ask for forms to be returned within 3 days of receipt.
  • Preschool, Pre-K and Kindergarten Teacher Questionnaire
  • Grades 1-8 Teacher Questionnaire  
• Parish Membership Letter from non-SLDM Catholic parish, if applicable (for tuition rate distinction - only required for families with students in grades K-8)

Your application is considered complete once all of the above listed documents are received by the school, including the completed Teacher Questionnaire from your child's current teacher.

If we have more applications that spots available in a given class, the Administration will follow the admissions criteria process listed on page 7 of the family handbook.

Notification of your child’s enrollment status will occur on January 30, 2026. Applications received after January 30th will be based on availability and you will be contacted within a week after receipt.

The Fall Open House in November kicks off the application process for the following school year. Tours are given and the online application link for the following year is made available at that time.
