First Reconciliation & First Communion

If you have a child desiring to receive the sacraments of First Reconciliation and First Holy Communion, at St. Louis de Montfort parish, please submit the registration form above. This applies to all parish families, including those whose children attend the parish school. You must submit a completed form in order for your child to receive the Sacraments.
